Solaris Healthcare Parkway is currently seeking a full-time Registered Nurse to fill the role of MDS Coordinator. The ideal candidate will be dedicated to providing excellent life and care services for our residents. The MDS Coordinator will be responsible for utilizing the federally mandated assessment tool to identify the needs and concerns of residents, as well as coordinating a care plan to support residents in achieving optimal wellness based on the highest practicable level of functioning.
We are located at 800 SE Central Pkwy, Stuart, FL 34994.

MDS Coordinator Qualifications:
- MUST POSSESS Registered Nurse RN License in Florida
- Organizational Skills/Ability to Multitask
- Knowledge of PDPM/Clinical Concepts
- Ability to work with facility nursing management to identify and fix care documentation notes
