Job Description: Working under the direction and supervision of the Nurse Manager, the LPN PRN LTC takes responsibility for the total care of residents on a team. They are responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care. The LPN PRN LTC works in collaboration with other healthcare professionals, residents, and their families, and functions as a role model for staff. They recognize the value of each person as an individual and support and promote resident strengths and abilities, capacity for social contribution, unique values, preferences, living habits, and autonomy of choice. An LPN PRN LTC at Holy Redeemer embraces a vision of aging that is meaningful and relationship-based.
Recruitment Requirements:
- Current LPN license
- Two years of LPN experience in a Long Term Care setting required
- Demonstrates positive communication skills and ability to relate to staff in a professional manner; able to relate to all levels of staff
