Job Description

Serenity Healthcare is seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will have the opportunity to make a positive impact on the lives of clients facing various challenges. You will work closely with individuals and families to assess their needs, provide counseling and support, and connect them with appropriate resources and services.

Key responsibilities for this role include:

• Conducting comprehensive assessments of clients' strengths, needs, and goals
• Developing and implementing effective care plans tailored to meet individual needs
• Providing individual and group counseling to clients to address emotional, behavioral, and social issues
• Advocating for clients' rights and ensuring access to necessary resources and services
• Collaborating with interdisciplinary teams to coordinate and provide integrated care
• Educating clients and their families on available support networks and community resources
• Maintaining accurate and up-to-date client records and documentation
• Participating in case conferences, team meetings, and professional development activities

Qualifications:

• Master's degree in Social Work or a related field
• Valid state licensure as a Social Worker
• Proven experience working in a healthcare or social services setting
• Strong knowledge of social work theories, practices, and ethics
• Excellent communication and interpersonal skills
• Ability to work effectively with diverse populations
• Highly organized and detail-oriented
