Job Description: Nurse Practitioner

Abbeyville Skilled Nursing And Rehabilitation Center, a leading healthcare facility located in Lancaster, Pennsylvania, is currently seeking a compassionate and experienced Nurse Practitioner to join our dedicated team. As a Nurse Practitioner with us, you will play a crucial role in providing exceptional patient care and advancing our mission towards improving the quality of life for our residents.

Responsibilities:
- Collaborate with our team of healthcare professionals to develop and implement comprehensive patient care plans based on a holistic approach.
- Conduct thorough physical examinations and assess patients' medical histories to diagnose, treat, and manage acute and chronic illnesses.
- Order, perform, and interpret diagnostic tests, such as blood work and X-rays, to facilitate accurate diagnoses and treatment plans.
- Prescribe and administer appropriate medications, treatments, and therapies, ensuring patient safety and adherence to established protocols.
- Provide education and counseling to patients and their families, empowering them to actively participate in managing their health conditions.
- Collaborate with other healthcare providers, including physicians and specialists, to coordinate and optimize patient care across different disciplines.
- Document patient encounters accurately and efficiently, maintaining comprehensive medical records that comply with regulatory standards.

Qualifications:
- Valid Nurse Practitioner license and certification in the state of Pennsylvania.
- Master's degree in Nursing with specialization as a Nurse Practitioner.
- Proven clinical experience in primary care, geriatrics, or a related field.
- Exceptional interpersonal skills and the ability to communicate effectively with patients, families, and multidisciplinary healthcare teams.
- Strong critical thinking and problem-solving abilities, with a focus on evidence-based practice and patient-centered care.
- Proficiency in using electronic health records and other healthcare technologies.
