Kadima Rehabilitation & Nursing At Pottstown is seeking a highly skilled RNAC/INC to join our team. As an RNAC/INC, you will be responsible for conducting assessments, ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, and coordinating the care planning process. This position requires a dedicated individual with excellent clinical knowledge and strong attention to detail.

Key Responsibilities:

• Conduct resident assessments to gather information for MDS completion
• Ensure accurate and timely completion of MDS assessments
• Collaborate with the interdisciplinary team to develop individualized care plans
• Review and analyze resident medical records to identify care needs
• Provide education and guidance to staff regarding MDS requirements
• Participate in the Quality Improvement program to ensure compliance with regulations

Requirements:

• Valid and current RN license
• Minimum of 2 years of experience in MDS coordination
• Strong knowledge of MDS completion and coding guidelines
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Attention to detail and strong organizational skills
