As a Nurse Practitioner at our center, you will have the opportunity to make a profound impact on the lives of our patients and their families. You will work closely with our interdisciplinary team to develop and implement personalized care plans, provide symptom management and support, and ensure the emotional well-being of our patients.

Responsibilities:

• Conduct comprehensive assessments of patients' physical, emotional, and social well-being
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Provide support and education to patients and their families regarding end-of-life care and decision-making
• Manage symptoms and provide palliative care to ensure patient comfort
• Coordinate and communicate effectively with the interdisciplinary team to ensure continuity of care
• Document patient care services accurately and timely in the electronic medical record system

Qualifications:

• Valid and unrestricted Nurse Practitioner license in the state of [State]
• Master's degree in Nursing from an accredited program
• Minimum of [number] years of experience as a Nurse Practitioner, preferably in hospice or palliative care
