Purpose of this position:
The primary purpose of the position is to maintain the RAI process, MDS and medical records, and process them through an electronic system of computers and related equipment in accordance with Federal and State guidelines, as well as our established policies and procedures, to assure that a complete medical records program is maintained. The MDS Coordinator may also function as a nurse on a unit.
Position Requirements:
Educational:
- Must possess, as a minimum, an ADN from an accredited college or university.
Licensure:
- Must possess a Nursing Degree from an accredited college/university
- A current unencumbered license to practice as an RN in FL.
- CPR certified
Experience:
- Must have computer skills and a working knowledge of medical terminology
- 5+ years
Job Functions:
- Complete PPS assessments in accordance with RAI Manual and CMS requirements
- Complete PPS, MDS/RAI, 5, 14, 30, and 60-day assessments as well as COT, EOT, et al.
- Complete Quarterly, Annual, and Significant change MDS/RAI
- Schedule and lead care conferences as directed. Attend standup, PPS, and other meetings as directed.
- Submission and transmission of MDS to state by electronic transfer via computer within 14 days of completion.
- Participate in analytical review of the QI/QM and Casper reports to be shared at the QAPI meeting. Collaborate with MDS Lead and DON for process improvement.
- Update resident care plans according to professional standards and CMS requirements
- Attend ongoing professional development through onsite and offsite programs
- Create opportunities to educate staff on MDS sections.
- Maintain fall and wound log
- Communicate diagnosis codes to medical providers to ensure proper billing in a timely manner.
- Complete chart audits specific to care plans, quarterlies, and assessments.
- Supervise restorative program and lead meetings with restorative aides.
- Collaborate to promote a value-based climate/culture with Family/Person-Centered Care.
- Obtain AANAC MDS Certification within one year of hire and maintain AANAC certification
- Use all appropriate written and electronic resources to ensure accuracy and completeness of MDS assessments
- Maintain the highest level of integrity and fidelity of MDS assessments, interviews, and care plans
- During periods of reduced census or as practicable, assist with staff education and/or admission process as directed
- Collaborate as part of the interdisciplinary team
