RN Manager - Care Management

Trinity Care Center is seeking a highly skilled and experienced RN Manager in Care Management to join our team. In this role, the RN Manager will be responsible for overseeing the care management services provided to our residents. The successful candidate will play a critical role in enhancing the quality of care and ensuring that our residents receive the support and services they need.

Key Responsibilities

• Provide leadership and guidance to a team of care managers to ensure the delivery of high-quality care services
• Develop and implement care plans that meet the specific needs of each resident
• Collaborate with healthcare professionals, including physicians and other specialists, to ensure effective care coordination
• Evaluate the effectiveness of care management programs and recommend improvements as necessary
• Perform regular assessments to monitor residents' health status and make appropriate care recommendations
• Ensure compliance with all regulatory guidelines and standards in healthcare
• Participate in the recruitment, training, and supervision of care management staff
• Maintain accurate and up-to-date documentation of residents' care plans and progress

Qualifications

• Valid Registered Nurse (RN) license in the state
• Proven experience in care management, preferably in a long-term care or skilled nursing facility
• Strong leadership and interpersonal skills
• Excellent communication and problem-solving abilities
• Knowledge of relevant healthcare regulations and guidelines
• Ability to work collaboratively in a team environment
