Job Description

We are currently seeking a passionate and dedicated Memory Care Facilitator to join our team. As a Memory Care Facilitator, you will play a crucial role in the lives of our residents by providing personalized care and support to enhance their overall well-being. Your main responsibility will be to facilitate various activities and programs specifically designed to stimulate memory and cognitive functioning.

Responsibilities:

• Develop and implement individual care plans for residents with memory impairments, tailoring activities to their specific needs and preferences.
• Lead group activities and one-on-one sessions to engage residents in stimulating cognitive, physical, and social exercises.
• Create a welcoming and supportive environment for residents, encouraging socialization and fostering connections among them.
• Document residents' progress, behaviors, and participation in activities to track and evaluate their cognitive function and overall well-being.
• Collaborate with the interdisciplinary team, including healthcare professionals and family members, to ensure the delivery of holistic care.
• Assist in organizing and coordinating outings, events, and celebrations for residents to enhance their overall experience at our facility.
• Stay updated on the latest research and approaches in memory care to continuously improve your skills and knowledge.

Qualifications:

• High school diploma or equivalent; associate degree or higher in a related field is preferred.
• Prior experience working with individuals with memory impairments, preferably in a senior care or memory care facility.
• Strong interpersonal skills and the ability to effectively communicate and connect with residents, families, and colleagues.
• Empathy, patience, and compassion for individuals living with memory loss.
• Understanding of dementia-related conditions and knowledge of person-centered care approaches.
• Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced environment.
• Flexibility to work on weekends, evenings, and holidays as needed.
