Job Description

We are currently seeking a passionate and dedicated Social Worker to join our team at Cornwall Manor. As a Health Center Social Worker, you will be responsible for providing social services to our residents in a long-term care setting. This is a full-time position, Monday-Friday, with some flexibility.

Responsibilities

As a Social Worker at Cornwall Manor, your responsibilities will include:

• Conducting individual and family psycho-social interviews
• Assisting with discharge planning
• Participating in care planning
• Completing PPS/MDS assessments
• Documenting social services in a long-term care setting

Qualifications

To be successful in this role, you should have:

• A Bachelor's degree in Social Work from an accredited institution
• Minimum of one-year social service experience in long-term care (preferred)
