Regional RN MDS Coordinator

The Grand Rehabilitation And Nursing At Batavia is seeking a highly skilled and motivated Regional RN MDS Coordinator to join our dynamic team. As a Regional RN MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments and facilitating communication and coordination between the nursing department, medical providers, and other healthcare professionals.


Responsibilities:

- Coordinate and oversee the completion and submission of MDS assessments according to federal and state regulations.

- Collaborate with interdisciplinary team members to collect and analyze patient data, ensuring accuracy and completeness.

- Review medical records, conduct observation, and interview patients to gather relevant information for MDS assessments.

- Ensure adherence to established standards, policies, and procedures related to MDS assessments and documentation.

- Serve as a resource for nursing staff, providing education and guidance on MDS assessment processes and requirements.

- Provide support and guidance to facility administrators and nursing directors in maintaining compliance with MDS regulations.

- Participate in quality improvement initiatives and audits to identify areas for improvement and implement corrective actions.

- Stay updated on changes in MDS regulations and related healthcare policies to ensure compliance and provide appropriate guidance.

- Collaborate with facility leadership to develop and implement strategies for optimizing reimbursement through accurate and comprehensive MDS assessments.


Qualifications:

- Registered Nurse (RN) license in the state of New York.

- Minimum of two years of experience in MDS coordination in a long-term care setting.

- In-depth knowledge of MDS rules and regulations, RAI process, and CMS guidelines.

- Strong attention to detail and ability to analyze and interpret complex data.

- Excellent communication and interpersonal skills, with the ability to effectively collaborate with interdisciplinary team members, residents, and families.

- Proficient computer skills, including working knowledge of MDS software and electronic medical records.

- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
