Job Description:
Pearl Of Rolling Meadows is currently seeking a skilled and compassionate Restorative Nurse to join our team. As a Restorative Nurse, you will play a vital role in helping our residents regain and maintain their optimal level of physical and psychological functioning.
In this position, you will work closely with our interdisciplinary team to develop and implement individualized restorative care plans for our residents. You will provide direct care, monitor progress, and make necessary adjustments to ensure the highest quality of care and outcomes.
Your responsibilities will include:
1. Conducting assessments: You will assess each resident's physical abilities, limitations, and areas of improvement to develop personalized restorative care plans.
2. Planning and implementing care: Based on the assessments, you will develop comprehensive care plans tailored to each resident's needs. You will coordinate with other healthcare professionals and communicate the care plan to the nursing staff.
3. Providing direct care: You will assist residents with exercises, mobility training, and other individualized interventions. You will closely monitor their progress and adjust the care plan as necessary.
4. Collaborating with the interdisciplinary team: You will work closely with nurses, therapists, and other healthcare professionals to ensure a holistic approach to resident care. Your input will be essential in updating care plans and achieving optimal outcomes.
5. Educating and training: You will provide guidance and education to residents, families, and staff on restorative care techniques and methods. Through regular training sessions, you will ensure staff members are equipped with the knowledge and skills to support residents' rehabilitation and wellness.
6. Documenting and reporting: You will maintain accurate records of resident progress, care provided, and any changes in condition. You will also contribute to regular progress reports and participate in care conferences.
7. Ensuring compliance: You will adhere to all applicable healthcare regulations and organizational policies to maintain the highest standards of resident care and safety.
Requirements:
- Valid state nursing license
- Proven experience as a Restorative Nurse or similar role
- Knowledge of restorative care principles and practices
- Strong communication and interpersonal skills
- Ability to work effectively within an interdisciplinary team
- Excellent observation and critical thinking abilities
- Strong organizational and time management skills
- Compassionate and patient-centered approach to care
