Social Worker

The Watermark at Logan Square, a leading retirement community in Philadelphia, is seeking a dedicated and compassionate Social Worker to join our team. As a member of our team, you will play a vital role in ensuring the highest level of healthcare and guest service for our residents.

Key Responsibilities:

- Provide social work services to residents across all levels of care in our large CCRC.

- Assist residents in navigating long-term and skilled nursing environments.

- Collaborate with interdisciplinary teams to develop and implement care plans.

- Support residents and their families in the discharge planning process.

- Maintain accurate and thorough documentation of assessments, interventions, and progress.

Requirements:

- Minimum of 2 years of experience as a social worker in long-term/skilled nursing environments.

- State-issued license in social work.

- Excellent knowledge of computer programs, including Microsoft Office.

- Strong written and verbal communication skills.

- Ability to work well within a team and contribute to our culture.

- Superior time management skills and attention to detail.

- Experience with discharge planning preferred.
