Position: Senior Registered Nurse - Coordinated Care Clinic

We are currently seeking a skilled and experienced Senior Registered Nurse to join our Coordinated Care Clinic. As a Senior Registered Nurse, you will play a vital role in coordinating patient care, managing a team, and ensuring the highest quality of healthcare services.

Responsibilities

• Oversee and manage daily operations of the Coordinated Care Clinic
• Coordinate and ensure delivery of patient care services
• Provide leadership and guidance to the nursing team
• Collaborate closely with healthcare professionals to develop and implement care plans
• Monitor patients' progress and make appropriate adjustments to care plans
• Educate and support patients and their families on healthcare issues and self-care
• Ensure compliance with healthcare regulations and standards
• Maintain accurate and detailed patient records
• Participate in quality improvement initiatives and contribute to the development of best practices

Requirements

• Bachelor's degree in nursing (BSN)
• Valid Registered Nurse (RN) license
• Minimum 5 years of experience as a Registered Nurse
• Strong clinical knowledge and skills
• Excellent leadership and interpersonal skills
• Ability to work effectively as part of a multidisciplinary team
• Proficient in electronic medical record (EMR) systems
• Excellent communication and organizational skills
