Job Description:

Hamilton Arms Center is currently seeking a Full-Time Day Admissions Coordinator (Social Worker) to join our team. As an Admissions Coordinator, you will play a crucial role in the admission process, ensuring the efficient and smooth transition of new patients into our facility.


Responsibilities:

• Conduct comprehensive assessments and evaluations of potential patients, gathering all necessary information for admission.

• Collaborate with patients, their families, and healthcare professionals to create personalized care plans and facilitate a seamless admission process.

• Coordinate with insurance providers to verify coverage and obtain authorization for admission.

• Conduct thorough and accurate documentation of patient information, assessments, and biopsychosocial histories.

• Serve as a liaison between the facility and external agencies, ensuring effective communication and timely resolution of any concerns.

• Participate in multi-disciplinary team meetings and provide valuable input to enhance patient care and outcomes.

• Stay updated with industry regulations and best practices to ensure compliance and provide the highest quality of care.

• Maintain a positive, compassionate, and professional demeanor when interacting with patients, families, and staff members.

• Perform other duties as assigned by the Admissions Director.


Requirements:

• Bachelor's degree in Social Work or related field.

• Valid state Social Worker license.

• Previous experience in healthcare or social work, preferably in an admissions role.

• Strong assessment and evaluation skills, with the ability to gather and analyze relevant information.

• Excellent communication and interpersonal skills, with the ability to effectively collaborate with diverse individuals and groups.

• Proficient computer skills, including MS Office and electronic medical records systems.

• Ability to prioritize and manage multiple tasks efficiently in a fast-paced environment.

• Strong attention to detail and strong organizational skills.
