Job Title: Full Time RNAC

Homestead Village, Inc. is seeking an experienced Full Time RNAC (Registered Nurse Assessment Coordinator) to join our team. As the RNAC for our 58-bed Skilled Nursing Facility, you will play a vital role in coordinating and providing exceptional nursing services to our residents.

Job Responsibilities:

• Assist and support fellow nurses in coordinating all nursing services in accordance with SNF Rules & Regulations, Resident Bill of Rights, Nursing Services P&P, and the HV Employee Handbook
• Provide supervision to staff through effective communication, education, coaching, and corrective action
• Co-Manage Wound Care and Infection Control services in SNF
• Co-Manage MDS Scheduling and Coordination with Medicare A (small), Medicaid, and Private Pay population
• Update care plans quarterly and as needed, follow up on care plan, monitor restorative nursing program completion
• Maintain 672 and 802 forms for assigned households
• Participate in the On-Call rotation

Qualifications:

• Current PA Registered Nurse license
• Bachelor’s degree in nursing, preferred
• Previous RNAC experience preferred or previous experience in a related position
• CPR Certified or ability to obtain
• Excellent computer skills, including EMR & MDS
• Knowledge of LTC regs & operations, including Federal, State, & Local regulations
• Demonstrated leadership and supervisory skills
• Effective and professional verbal/written communication with all stakeholders
• Ability to effectively educate and train co-workers on clinical and non-clinical topics
• Knowledge of infection control regulations and procedures
• Knowledge of wound prevention, assessment, and treatment
• Adheres to the Company Wide Behaviors

Experience:

Minimum of 1 year of experience as an RNAC (Registered Nurse Assessment Coordinator) and in Long Term Care

License/Certification:

Active RN license in the state of Pennsylvania
