Job Description
The Iowa Department of Health and Human Services’ (Iowa HHS) Division of Public Health, Bureau of Chronic, Congenital and Inherited Conditions is seeking a Family Support Coordinator (Program Planner 1).
Summary:
This position provides follow-up for families with children who did not pass their newborn hearing screen or who were diagnosed with hearing loss or deafblind. Follow-up includes providing in-person and group support opportunities, assisting families navigate the healthcare journey and communicating with families via virtual methods or in-person opportunities.
Desired Qualifications:
Preferred but not required: Candidates with lived experience as a parent of a deaf or hard-of-hearing child or a parent of a blind child or a parent of a deafblind child.
Responsibilities:
Provide support and follow up for families through group and individual communication methods and meetings. Track data for family resource needs and referral outcomes. Develop new and strengthen existing relationships and communication methods with providers and families.

Minimum Qualification Requirements
Graduation from an accredited college or university with a Bachelor's Degree;
OR
A total of four years of education and/or full-time experience, where one year of full-time experience in the administration or operation/execution (i.e., providing consultation and/or technical assistance to stakeholders, research and analysis, or enforcement of program guidelines/policies/procedures) of a program (i.e., the provision of a service or an administrative oversight/enforcement responsibility) equals thirty semester hours of accredited college or university course work in any field.
OR
employees withcurrentcontinuous experience in the state executive branch that includes the experience equal to eighteen months of full-time work as a Planning Aide.
