Essential Functions:
- Seeks and identifies way to support resident’s individual needs and preferences, customary routines, concerns and choices. Communicates this information to immediate supervisor.
- Carries out individual and group activities that meet the functional levels, needs and interests (including cultural and religious).
- Provides activities for residents confined to their rooms that reflect life-long interests and that are appropriate to the individual’s functional abilities and level of participation.
- Facilitates household meetings on designated floors, acts as liaison to facilitate residents’ requests, corrects concerns while working with other departments to accomplish requests. Documents process for State and Federal compliance.
- Adapts activities to match the cognitive and physical functional levels of the residents through the use of task segmentation, verbal prompts, set-up assistance, physical assists and demonstrations.
- Assists Director of Activities to develop activity calendars. Assures distribution of schedules to residents.
- Monitors physical environment for safety or fire hazards and corrects or reports potential problems immediately to unit charge nurse. Promptly reports housekeeping and maintenance problems to facilities department.
- Replaces all activities equipment and supplies in the proper storage areas.
- Records progress notes in the clinical records both objective and subjective information as it relates to behavior and residents’ plan of care at prescribed intervals. Able to complete are required assessments on MDS reports in both written and electronic formats.
- Electronically record all participation and behavior observations.
Position Requirements:
- High school diploma or equivalent.
- One year supervised experience in a health care setting providing therapeutic activities to individuals.
- Sufficient education to demonstrate spoken and written English.
- Demonstrates strong communication and relationship building skills with residents, and is able to explain residents’ needs to others as needed for resident advocacy.
- Specialized training or experience in geriatric care or gerontology is desirable.
