Primary Care and Hospital Medicine Physician

JOB OPPORTUNITY: Primary Care and Hospital Medicine Physician Hartford HealthCare Medical Group, Inc. (“HHCMG”) seeks a full-time Primary Care and Hospital Medicine Physician to: Perform comprehensive assessments, treatment, and symptom and disease management of adult outpatients and inpatients with a variety of medical complaints and diagnoses; provide patient and family education and support; document patient information in the electronic patient record according to medical group policy and regulatory requirements; participate in quality improvement and practice development programs for Primary Care Office (“PCO”) and hospitals; provide leadership in committees and task forces and participates in educational activities related to hospital and primary care medicine services; admit patients in accordance with hospital patient acceptance policies; work with Emergency Department physicians and other specialty services to assure effective communication and transition of patients to optimize their care; work with other specialty services to assure highest quality of patient care through consultations to and from specialty services; and make referrals to community physicians. Spends 60% of work time with PCO outpatients and 40% with hospital inpatients. Works at HHCMG's PCO, 42 Town Street, Norwich, CT 06360; Backus Hospital, 326 Washington Street, Norwich, CT 06360; and Windham Hospital, 112 Mansfield Ave, Willimantic, CT 06226. The minimum requirements for the Primary Care and Hospital Medicine Physician position are: U.S. M.D. or D.O. degree or the foreign equivalent; completion of three-year residency training program in Internal Medicine or Family Medicine; board eligibility for or certification in Internal Medicine or in Family Medicine; and possession of or eligibility for an unrestricted Connecticut medical license.
