Job Description

Story Medical Senior Care is currently seeking a dedicated and compassionate Social Worker to join our team in Cambridge, Iowa, located in Story County, IA. As a Social Worker, you will provide professional services to clients and their families, meeting their identified psychosocial, emotional, financial, and environmental needs.

Responsibilities:

• Conduct psychosocial assessments to understand the individual needs of clients and families.
• Provide supportive counseling to clients and their families.
• Offer emergent crisis intervention when necessary.
• Assist clients in accessing community resources and services.
• Collaborate with healthcare professionals and other members of the interdisciplinary team.
• Advocate for clients' rights and best interests.
• Stay up-to-date with relevant laws, regulations, and best practices in social work.

Requirements:

• Master's degree in Social Work or related field.
• Valid state Social Work license.
• Previous experience in a healthcare or senior care setting is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.
• Compassionate and empathetic approach to client care.
• Knowledge of community resources and services.
