Registered Nurse Assessment Coordinator (RNAC)-in-Training
Guardian Healthcare Highland View is currently seeking a Registered Nurse Assessment Coordinator (RNAC)-in-Training.
The RNAC-In-Training will work and train with Registered Nurse Assessment Coordinator (RNAC) to acquire the skills and knowledge necessary to be a successful RNAC. The RNAC-In-Training will gather MDS data via the Resident Assessment Instrument (RAI), which assesses a resident's behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan that considers the resident holistically. The RNAC-In-Training works with nursing, therapy, nutrition, social services, and activities to ensure care plan guidelines for each resident are followed, and compliant with ethical standards and Medicare requirements. The RNAC-In-Training understands and follows all current state, federal, and CMS regulations.
This is a full time position. Each week; 2 days will be spent in the training program and 3 days will be spent working as a floor nurse.

What will you do as a Registered Nurse Assessment Coordinator (RNAC)-in-Training
- Trains with assigned RNAC on case management policies and procedures.
- Provide direct nursing care to the residents
- Supervise the day-to-day nursing activities performed by nursing assistants
- Interact and communicate with the family members of our residents in a patient and professional manner.
