Job Summary

You will deliver comprehensive, patient-centered care to patients and residents at a skilled nursing/long-term care facility and coordinate with partnering hospital clinicians where applicable.

APP Job Duties:

APP will be responsible for rounding on short-term rehab patients and long-term care patients in the skilled nursing or assisted living setting.

Flexible schedule, in-house hours: M-F approximately.

On-call responsibility with rotating schedule for after-hours and weekends.

Care coordination support for scheduling and visit tracking.

Timely chart completion responsibility.

Participation in important quality of patient care or family meetings, psychotropic, morning meeting, and/or others when needed or asked.

Maintain good working relationships and strong communication with the Medical Director/attending physician.
