MDS Nurse (RN) - Crowne Health Care of Springhill

Job Description

Crowne Health Care Of Mobile is currently seeking a highly skilled and experienced MDS Nurse (RN) to join our team at Crowne Health Care of Springhill. As an MDS Nurse, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessment process for our residents.

Your primary responsibilities will include:

• Conducting and coordinating comprehensive assessments of each resident
• Ensuring accurate and timely completion of MDS assessments in accordance with state and federal regulations
• Collaborating with interdisciplinary team members to develop individualized care plans
• Regularly reviewing and updating care plans based on residents' needs and changes in condition
• Monitoring and documenting resident progress and outcomes
• Participating in quality improvement initiatives and ensuring compliance with all applicable regulations and guidelines
• Providing education and training to staff on MDS-related topics

Qualifications

To be successful in this role, you must meet the following requirements:

• Valid Registered Nurse (RN) license in the state of Alabama
• Minimum of 2 years of nursing experience, preferably in a long-term care setting
• Strong knowledge of MDS requirements and regulations
• Excellent assessment and documentation skills
• Ability to work effectively within a team and independently
• Attention to detail and strong organizational skills
