Job Description

Transitions Healthcare North Huntingdon, a leading healthcare facility in Irwin, PA, is seeking an experienced and motivated individual to join our team as an Admissions Coordinator. Responsibilities

  • Coordinate resident/patient referral and approval process in a timely manner
  • Review referrals and complete admissions meetings
  • Conduct tours of the facility with potential residents/patients and their family members or POAs
  • Track pre-admission referral activity daily/weekly and present information at daily morning meetings
  • Attend daily census meeting and provide admission information as needed
  • Assist with the arrangements and confirmation of all admissions and completion of required paperwork within 48 hours of admission (contingent on family availability)
  • Notify departments of anticipated admission
  • Complete daily/as needed updates to Admission board
  • Contact hospital social worker within appropriate timeframe to provide acceptance/decline of referral
  • Assign room number and attending physician to all admissions and provide Admission/Financial information to physician’s offices
  • Accurately input initial Admission information into PCC (within 1 hour before patient arrival)
  • Provide initial Admissions paperwork to appropriate nursing staff prior to resident arrival
  • Provide Business Office with Admission paperwork and insurance cards within appropriate time frame
  • Complete information emails to Transitions employees to indicate resident hospital returns
  • Contact resident families regarding hospital bed hold policy and update information in census report
  • Inform facility management team and Hospital social worker of Bed Hold information obtained from families
  • Monitor residents on hospital bed hold by reviewing doctor progress notes/nursing notes in the Allscripts or Cerner systems and contacting other hospital social workers (after day 3 of bed hold)
  • Promptly communicate specialized equipment needing to be ordered as indicated (verify payor coverage - obtaining prior approval from authorized payor when needed)
  • Assist with Accounts Payable function

Qualifications

  • Minimum high school diploma or equivalent required
  • Excellent computer skills in a Microsoft Windows environment
