Job Description

We are currently seeking a passionate and dedicated Social Worker to join our team as a full-time day staff member. As a Social Worker at Elizabeth Seton Pediatric Center, you will play a vital role in improving the lives of our young patients and their families.

Your responsibilities will include:
- Conducting comprehensive psychosocial assessments to determine the needs and strengths of patients and their families
- Developing individualized care and discharge plans in collaboration with interdisciplinary teams
- Providing supportive counseling to patients and families to address emotional and behavioral challenges
- Advocating for patients and families to ensure access to necessary resources and services
- Collaborating with community agencies and organizations to coordinate post-discharge care and support
- Documenting assessments, interventions, and outcomes accurately and timely
- Participating in case conferences, team meetings, and continuous education opportunities to enhance knowledge and skills

Requirements

We are looking for candidates who meet the following qualifications:

- Master's degree in Social Work (MSW) from an accredited university
- Current state licensure as a Licensed Clinical Social Worker (LCSW)
- At least 2 years of experience in pediatric social work, preferably in a healthcare setting
- Knowledge of relevant laws, regulations, and best practices in pediatric social work
- Strong communication, interpersonal, and problem-solving skills
- Ability to work collaboratively with interdisciplinary teams
- Excellent organizational and time management abilities
- Compassionate, empathetic, and patient-oriented attitude
