Claremont Nursing & Rehabilitation Center is currently seeking a dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing support and services to our residents and their families.

Responsibilities:

• Assess the psychosocial needs of residents and provide appropriate interventions and support.
• Collaborate with healthcare professionals, including doctors, nurses, and therapists, to develop and implement resident care plans.
• Assist residents and their families in understanding and accessing available resources, such as Medicare, Medicaid, and other financial assistance programs.
• Coordinate and facilitate communication and coordination between residents, their families, and healthcare providers.
• Advocate for residents' rights and ensure their emotional well-being is prioritized.
• Provide counseling and support to residents and their families during times of transition, grief, and crisis.
• Maintain accurate and up-to-date documentation of all interactions and activities.

Qualifications:

• Bachelor's or Master's degree in Social Work from an accredited institution.
• Licensed as a Social Worker in the state of {insert state} (if applicable).
• Prior experience in a long-term care or healthcare setting preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively as part of a multidisciplinary team.
• Empathy, compassion, and a commitment to providing exceptional care and support.
• Knowledge of relevant regulations and laws pertaining to long-term care and social work practice.
