Position Overview

We are currently seeking a skilled and compassionate Social Service Designee to join our team. In this role, you will be responsible for assisting residents in accessing social services and ensuring their overall well-being. You will work closely with residents, their families, and other healthcare professionals to develop and implement individualized care plans.

Responsibilities

As a Social Service Designee at Oaks of Brecksville, your responsibilities will include:

• Assisting residents in understanding and accessing social services available to them
• Assessing the social and emotional needs of residents and developing care plans accordingly
• Collaborating with interdisciplinary teams to ensure the comprehensive care of each resident
• Providing counseling and support to residents and their families
• Advocating for residents' rights and ensuring their voice is heard
• Maintaining accurate and up-to-date documentation of resident interactions and progress

Qualifications

To be successful in this role, you should possess the following qualifications:

• Bachelor's degree in Social Work or a related field
• State certification as a Social Service Designee
• Experience working with seniors and a strong understanding of their unique needs
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
