MDS-LVN

SUMMARY

Provides assistance in Resident care planning activities, including the MDS and MMQ processes. Participates in timely completion of the MDS process (Minimum Data Set, discharge, and re-entry tracking forms).

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assists the Resident Assessment Coordinator in fulfilling responsibilities, as directed.
• Completes assessments of Residents that directs the plan of care.
• Conducts orientation sessions on the MDS process to all employees involved.
• May coordinate information systems operations and education for the clinical department.
• Responsible for contacting Medicare benefits hotline as required per company guidelines.
• Develops schedule of due dates for periodic assessments.
• In the absence of the Resident Assessment Coordinator, may perform duties of that position with oversight by a Registered Nurse (if applicable).
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

ENTRY QUALIFICATIONS

• Graduate of an accredited school of Nursing.
• Valid State registration as an LPN/LVN.
• Knowledgeable of OBRA, Perspective Payment System, and MDS process.

Starting Rate of $38.00 Negotiable upon experience.
