Care Management Support Professional - Prescription Assistance Program

Bethesda is currently seeking a dedicated Care Management Support Professional to join our Prescription Assistance Program team. In this role, you will play a vital role in assisting patients with their prescription medication needs, ensuring they have access to the medications they require to maintain their health and well-being.

Responsibilities

- Collaborate with healthcare providers, pharmacies, and insurance companies to facilitate the prescription assistance process

- Educate patients on available prescription assistance programs and help them navigate the application and enrollment process

- Conduct assessments to determine patients' eligibility for prescription assistance programs

- Maintain accurate records of patient interactions and program eligibility

- Provide ongoing support to patients, addressing any questions or concerns they may have

- Assist in the coordination of medication delivery and refill processes

Qualifications
