ACC Care Center is looking for an Admissions Assistant who is dedicated to serving the Sacramento senior community. ACC Care Center is a skilled nursing facility that provides wound care, short-term rehabilitation, and daily living care for long-term residents. GENERAL Summary:
The Admissions Assistant assists in the admissions process and helps ensure that new residents are integrated seamlessly into the system.
ESSENTIAL JOB FUNCTIONS:
- Schedules tours for admission inquiries.
- Provides advance notification to department heads regarding appropriate preparation and follow-up for admissions (both new admissions and re-admits) and communicates questions or concerns to the appropriate department manager or Administrator.
- Receives and reviews inquiries regarding the facility via telephone, email, fax, and in person.
- Extracts information from referrals and completes information sheet for team review.
- Assists in making arrangements with discharge planners, families, and physicians to accomplish admission to the facility.
- Collects and communicates information relative to resident admissions and performs administrative tasks in the electronic medical record to document the resident's admission status. This may include preparing admissions paperwork for signatures and providing/completing admission packets (e.g., resident rights, a notice of privacy practices, admissions contract, etc.).
- Assists in the resident admission orientation to the facility/room per established policies and procedures, including the provision of the welcome box, an inspection of the resident's room, and escorting residents to their room.
- Obtains all necessary cards to verify the identity and financial status of the resident (i.e., HMO, Medicare, or Medi-cal cards) and verifies entitlement to HMO, Medicare, etc., coverage and completes costing of patient.
- Checks Common Working File for Insurance/Payment accuracy.
- May assist the Admissions Coordinator and Billing Manager in providing explanation to the resident and/or responsible party about the room rates, billing procedures, visitors'/guest privileges, restrictions, resident care procedures, etc., as appropriate.
- Refers admissions concerns to proper person, I.e., Admissions Coordinator, Business Office Manager, nursing, social services, etc.
- Ensures residents' valuables are inventoried and deposited or a release from responsibility form is signed by the established policies and procedures.
- Maintains daily and monthly census report as directed and emails the daily census information to the required parties before 9:00 a.m. each weekday.
- Prepares and maintains weekly referral reports following the company standards for discharges, admissions, and referral sources.
- Maintains a current listing of all residents and assigned room numbers. Provide copies of such listing to nursing, management, government agencies, etc., as necessary and following the privacy rules.
- Maintains documentation of discharge dates and times from the facility.
- Maintains residents' waiting list according to established policies and procedures.
- Follows up on all pending admissions daily or as directed by the Admissions Coordinator.
- Issues Medicare denial letters as directed by the clinical navigators.
- May serve as Receptionist for the facility and provide administrative support to other departments.
- Reports the following established facility procedures and regulatory standards: accidents and incidents; resident grievances, complaints, allegations of resident abuse and/or misappropriation of resident property; occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals; concerns and suspected incidences of non-compliance including but not limited to hazardous conditions and equipment.
- Performs other related duties as required.
MINIMUM QUALIFICATIONS
- Two years of office work experience preferred.
- High School graduate or equivalent.
- Effective communication skills, both verbal and written.
- Multilingual is a plus.
- Excellent organizational skills.
- Demonstrated proficiency with Microsoft Office Suite, i.e. Word and Excel.
- Ability to work compassionately with residents and families, while committing to a positive organizational culture based on mutual respect, cooperation, and openness to others' needs and perspectives.
