Case Manager - Full-Time, Veterans Honor Home

As a Case Manager at the Ohio Veterans Home's Veterans Honor Home, you will play a crucial role in ensuring the well-being of our residents. In this full-time position, your primary responsibility will be to provide case management services to veterans and their families. You will work closely with the interdisciplinary team to develop and implement care plans, coordinate services, and advocate for the needs of our residents.

Responsibilities

• Conduct comprehensive assessments to evaluate the needs and goals of residents
• Develop and implement individualized care plans
• Monitor and evaluate the effectiveness of care plans
• Coordinate and facilitate communication between residents, families, and the healthcare team
• Provide support and guidance to residents and their families
• Advocate for the rights and dignity of residents
• Collaborate with community resources to ensure access to necessary services
• Maintain accurate and up-to-date documentation

Qualifications

• Bachelor's degree in social work or related field, or equivalent experience
• Knowledge of case management principles and practices
• Strong communication and interpersonal skills
• Ability to assess needs, develop care plans, and evaluate outcomes
• Experience working with veterans and/or older adults is preferred
• Understanding of military culture and the unique needs of veterans is a plus
• Proficient computer skills and experience with electronic medical records
