Responsibilities and Duties

As an MDS Coordinator, your primary responsibilities will include:

• Conducting comprehensive assessments of residents' physical, mental, and psychosocial needs
• Ensuring accurate and timely completion of MDS assessments
• Collaborating with interdisciplinary team members to develop individualized care plans
• Overseeing the implementation and evaluation of care plans
• Ensuring compliance with all applicable regulations and standards

Qualifications and Skills

To be successful in this role, you must possess the following qualifications and skills:

• Valid Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in the state of employment
• Prior experience in MDS coordination in a skilled nursing facility setting
• Strong knowledge of MDS assessment process and regulations
• Excellent communication and interpersonal skills
• Attention to detail and ability to maintain accurate records
• Ability to effectively coordinate care and collaborate with interdisciplinary team members
• Proficient computer skills
