About the Role: Lemay Avenue Health & Rehab, located in Fort Collins, is hiring a quality LPN-Licensed Practical Nurse who plays an important role in the recovery and well-being of our post-acute and elderly patients and residents.
Starting wages: $27.00 - $40.00/hr. base pay based on years of experience, plus shift differentials! Evening differential $3.00, Overnight differential $4.00, Weekend shift differential $3.00, Retention bonus, Flexible scheduling, Scholarships for career pathways, and educational reimbursement.

Work Schedule: Full-time, Part-time or PRN shifts, Days: 6am-2:30pm, Eves: 2pm-10:30pm, Overnights: 10pm-6:30am, 8-hour and 12-hour shifts available. Holiday schedule - Time and a half paid.
Licensed Practical Nurse Responsibilities:
- Provides care to residents in a manner that enhances each residents' quality of life
- Performs delegated independent nursing functions according to each resident's plan of care using established procedures, policies, guidelines, and standards
- Administers prescribed medications and treatments accurately, observing resident response
- Collects samples, such as urine, blood, and sputum for residents
- Assists nursing assistants to attend to the individual needs of residents
- Monitors, Observes, reports and records physical, mental, and psychological changes
- All other duties as assigned
Qualifications: Nursing degree from an accredited college or university, Clear unencumbered license from the State of Colorado as a Licensed Practical Nurse, Current CPR Certification, New Grad LPNs Welcome as well as any licensed nurse within a hospital, long-term care or any other related healthcare facility.
