Job Summary

We are currently seeking an exceptional Social Services Director to join our team. In this role, you will have the opportunity to make a meaningful difference in the lives of our residents. As the Social Services Director, you will be responsible for overseeing and coordinating social services for our residents. This includes conducting assessments, developing care plans, providing counseling and support, and collaborating with internal and external stakeholders.


Job Responsibilities

  • Conduct assessments to identify residents' needs, strengths, and preferences
  • Develop individualized care plans in collaboration with residents and their families
  • Provide counseling and support to residents, addressing emotional, social, and psychological needs
  • Coordinate and monitor social services to ensure residents receive appropriate care and support
  • Collaborate with internal teams and external agencies to coordinate services and resources for residents
  • Maintain accurate and up-to-date documentation of social services provided
  • Stay informed about current social services regulations and best practices
  • Participate in interdisciplinary care conferences and meetings
  • Supervise and mentor social services staff

Qualifications

  • Bachelor's degree in Social Work or a related field
  • Licensure as a Social Worker (LSW, LISW) in the state of Ohio
  • Prior experience working in a social services role, preferably in a long-term care or senior living setting
  • Demonstrated knowledge of social services practices, regulations, and resources
  • Excellent communication and interpersonal skills
  • Strong organizational and time management abilities
  • Compassionate and empathetic approach to working with older adults
  • Ability to effectively collaborate with internal and external stakeholders
