Job Description

As the Social Services Director at Vale Healthcare Center, you will play a vital role in ensuring the emotional and social well-being of our residents. You will be responsible for coordinating and overseeing all social services activities within our facility.

Responsibilities

Your main responsibilities include:

Developing and implementing individual care plans for residents based on their social, emotional, and psychological needs
Providing counseling and support to residents and their families
Assessing resident's social needs, documenting their progress, and adjusting care plans accordingly
Coordinating with other healthcare professionals and community organizations to provide additional support and resources
Advocating for residents' rights and ensuring their needs are met
Collaborating with the interdisciplinary team to create a holistic approach to resident care
Providing education and training to staff members on social services-related topics
Maintaining accurate and up-to-date records of all social services activities

Qualifications

To be successful in this role, you should possess the following qualifications:

Bachelor's degree in social work or a related field
Previous experience in a social services role, preferably in a healthcare setting
Strong communication and interpersonal skills
Excellent organizational and time management abilities
Ability to work effectively in a multidisciplinary team
Knowledge of relevant laws, regulations, and ethical standards related to social work
Proficiency in computer skills and electronic documentation
