Job Description:
Minnesota Senior Health Options (MSHO) Care Coordinator duties include responsibility for assisting members enrolled in a dual eligible Minnesota Health Care Programs to make informed choices about their health care services, conducting assessments, service coordination, and assisting members in making progress towards his or her health goals and coordinating care with the interdisciplinary team. This includes making in-person visit to enrollees’ home.
Education Qualifications:
This is a hybrid position.
The candidate will work remotely from their home office, but they will be making visits in the community to persons living in Ely and the surrounding areas.
