Registered Nurse MDS Coordinator RNAC
The Delaware Valley Skilled Nursing & Rehabilitation C is currently seeking a motivated and experienced Registered Nurse MDS Coordinator RNAC to join our team. As an RNAC, you will be responsible for completing and submitting MDS assessments and other required reports. Your role will involve managing and coordinating the care plan process in accordance with HCFA guidelines, ensuring accurate and current information is provided for resident's care plans.
Key Responsibilities:
- Complete residents MDS Assessment Reports, annual & quarterly reviews, Raps, and MA 103 as needed
- Coordinate information for resident's care plans
- Ensure accurate, current information is provided to third-party payers for reimbursement
- Ensure the daily treatment/care provided to the resident is appropriate to their diagnosis and condition and meets physician's and therapist's recommendations
- Participate in IDT meetings for resident care and progress
- Serve as a member of the Quality Assurance Team and other appropriate committees as requested
- Attend and participate in staff meetings
Requirements:
- Valid RN license
Job Type: Full-time
Pay: From $85,000.00 per year
