The payments begin when the candidate starts and are disbursed over time. The contract terms are:

Part-Time Social Services Specialist

Walker Methodist Westwood Ridge in West St. Paul offers senior living with services to 127 customers and a 37-bed transitional care and rehabilitation center, 24 memory care apartments, and 10 care suites.

Location: Saint Paul, MN

Schedule: Part-Time - 24 hours per week

Requirements:

  • Education: Human Services or other related degree preferred. Recent graduate of Social Worker degree program.
  • Preferred Experience: Previous experience working in a medical setting. Previous experience working as a Social Worker or Case Manager.

Responsibilities:

  • Participate in contact with residents and their contacts/families in order to maintain professional relationships with them.
  • Provide an opportunity for them to offer suggestions, ask questions, address concerns, and support the resident and their contacts/families.
  • Coordinate resident moves within the building/community according to the Resident Transfer Policy.
  • Assist residents with mail as requested (whose mental functions are impaired). Review during care conferences or when change requested to personal and/or business mail and responsibilities. Maintain accurate process in PCC profile.
  • Facilitate care conferences with the interdisciplinary team. Encourage resident to direct meeting as to their greatest ability and independence.
  • Provide crisis intervention for residents and contacts/families when appropriate. Collaborate with additional services, psychological/psychiatrist services, outside mental health support, and therapy.
  • Awareness and supporting resident and families/contacts in obtaining guardianship, POA, and conservatorship services for residents in need of such services.
  • Assess social/psychological wellbeing needs of the residents and develop personalized, person-centered care plan that has specific, individualized goals, approaches, and actions.
  • Complete MDS, CAAs, and care plans in accordance with the RAI policy and procedures.
  • Document pertinent information in the integrated progress notes in the resident's medical record (PCC) and/or soft file.
  • Communicate effectively with residents, their contacts/families, other department staff and all other customers in order to provide direction, obtain information, promote interdisciplinary teamwork, encourage quality of life specific to each resident, and facilitate problem-solving.
  • Initiates discharge planning on the day of resident admission to the TCU and/or long-term care if desired.
  • Complete a Discharge Assessment within two (2) days of the resident's/patient's admission.
  • Notify the resident and responsible party when the discharge referral is made and when services will begin.
  • Complete assessments, MDS, and care plan in accordance with state, federal, and Walker Methodist requirements.
  • Other duties as assigned by supervisor.
