Job Description
Children's Hospital of The Kings Daughters Health System is seeking a highly compassionate, professional, and caring LPN - Primary Care Coordinator to facilitate health and healing for our patients and their families. This position is with Suffolk Pediatrics and maintains a typical schedule of Monday - Friday, 8:00AM - 5:00PM.
Responsibilities
The LPN - Primary Care Coordinator will be responsible for:
- Coordinating and providing comprehensive care to pediatric patients
- Assisting with program coordination and management
- Collaborating with healthcare professionals to ensure proper patient care
- Ensuring accurate documentation of patient information
- Assisting with patient education and support
- Performing clinical tasks as needed
Qualifications
The ideal candidate for this position will possess the following qualifications:
- Current State license as a Practical Nurse (LPN) or Practical Nurse holding a valid Compact State license
- Ability to travel
- 3 - 5 years clinical experience in a practice setting preferred
- Pediatric and program coordination experience preferred
- CPR training required or must be obtained within 90 days of hire
- Must provide a record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing
