Job Description

Children's Hospital of The Kings Daughters Health System is seeking a highly compassionate, professional, and caring LPN - Primary Care Coordinator to facilitate health and healing for our patients and their families. This position is with Suffolk Pediatrics and maintains a typical schedule of Monday - Friday, 8:00AM - 5:00PM.

Responsibilities

The LPN - Primary Care Coordinator will be responsible for:

• Coordinating and providing comprehensive care to pediatric patients
• Assisting with program coordination and management
• Collaborating with healthcare professionals to ensure proper patient care
• Ensuring accurate documentation of patient information
• Assisting with patient education and support
• Performing clinical tasks as needed

Qualifications

The ideal candidate for this position will possess the following qualifications:

• Current State license as a Practical Nurse (LPN) or Practical Nurse holding a valid Compact State license
• Ability to travel
• 3 - 5 years clinical experience in a practice setting preferred
• Pediatric and program coordination experience preferred
• CPR training required or must be obtained within 90 days of hire
• Must provide a record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing
