As the RN/Licensed Practical Nurse Assessment Coordinator, you will be responsible providing high quality care for the residents here at Concordia. Responsibilities include but not limited to responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and medical records for residents. Duties include coordinates, conducts, and completes the MDS, family/resident care plans and conferences on the computerized system, assures accuracy and completeness of medical records, and works with managed care companies to provide clinical updates and reviews. The licensed practical nurse must demonstrate leadership ability and have excellent written and verbal communication skills.

Competitive starting wage and shift differentials offered. This is a full-time position and include a weekend rotation.

Qualifications:

• Licensed Practical Nurse (LPN) or RN currently licensed by the state of Pennsylvania
• Two (2) years nursing experience preferred but not required
• Six (6) months of Geriatric nursing experience preferred but not required
• Must be self-motivated and willing to engage in not only hands-on care, but to follow-up with documentation compliance
• Must possess CPR and First Aid certificates or willing to obtain each within six (6) months of employment
• AANAC strongly encouraged
