Job Description

The Coordinator Admissions Marketing Registered Nurse-Skilled Nursing will play a vital role in assessing patients referred to the skilled nursing facility (SNF) for admission eligibility. Additional responsibilities include preparing patients and their families for discharge from acute care and admission to the SNF, participating in discharge reviews conducted by Care Coordination, initiating residents' admission to the SNF, and maintaining records for statistical purposes.

Job Duties

• Assesses patients referred to SNF from Care Coordination and outside hospitals for eligibility for admission.
  30% of Time
• Prepares the patient and family for discharge from acute care and admission to the SNF.
  25% of Time
• Participates in the discharge review conducted by Care Coordination on acute care patients ready for discharge.
  15% of Time
• Initiates residents' admission to SNF and notifies SNF of pending admission. Assists with the admission process at SNF.
  20% of Time
• Maintains records for statistical purposes and submits reports as assigned.
  5% of Time
• Performs other duties as assigned.
  5% of Time

Qualifications

• Education, Credentials, Licenses: Graduate of an approved school of nursing and licensed to practice nursing in the state where work is being performed. BSN preferred.
• Specialized Knowledge: Thorough knowledge of the CMS admission guidelines for SNF. Experience in a traditional LTC setting. Marketing experience a plus.
• Kind and Length of Experience: 2 years in aging services with Medicare skilled nursing background.
