- Assist residents in accessing community resources and benefits to enhance their quality of life
- Document social service activities clearly and accurately, maintaining confidentiality and adhering to regulatory requirements
- Stay informed about current social service practices and industry trends to continuously enhance our offering
Qualifications:
- Bachelor's degree in Social Work or related field
- State licensure or certification as required
- Prior experience in a social services role within a healthcare or long-term care setting preferred
- Strong interpersonal and communication skills, with the ability to build rapport and trust with residents, families, and colleagues
- Excellent problem-solving and decision-making abilities
- Knowledge of community resources and social service programs
