Position: MDS-LVN
We are currently seeking a detail-oriented and experienced MDS-LVN to join our team. As an MDS-LVN, you will play a key role in ensuring accurate and comprehensive assessments, care planning, and documentation for our residents. Your expertise and dedication will contribute to the overall well-being and quality of care provided at Caraday Mesquite.
Responsibilities:
- Conduct comprehensive resident assessments and develop individualized care plans.
- Utilize the Minimum Data Set (MDS) to gather information and document resident needs.
- Collaborate with the interdisciplinary team to ensure proper clinical documentation.
- Regularly review and update care plans based on ongoing assessments and changing needs.
- Adhere to all regulatory guidelines and standards.
- Communicate effectively with residents, families, and team members.
- Participate in care conferences and team meetings to discuss resident progress and goals.
- Provide guidance and support to direct care staff.
- Maintain accurate and up-to-date records.
Qualifications:
- Valid LVN license in the state of Texas.
- Minimum of 2 years of experience in MDS completion.
- Strong knowledge of Medicare, Medicaid, and insurance guidelines.
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Attention to detail and accuracy in documentation.
- Effective communication and interpersonal skills.
