Job Description:

Miami Jewish Health Systems is seeking a dedicated and compassionate individual for the position of Care Navigator, Integrated Rehab Program. As a Care Navigator, you will play a vital role in helping patients navigate through the integrated rehab services offered by our organization.

Responsibilities:

• Assist patients in understanding the available rehab services and help them make informed decisions about their care.
• Coordinate with interdisciplinary teams to develop and implement personalized care plans for patients.
• Advocate for patients and ensure their needs are met throughout the rehab process.
• Conduct eligibility screenings and facilitate referrals to appropriate programs.
• Collaborate with community organizations to enhance the continuum of care for patients.
• Provide ongoing support and education to patients and their families on available resources and services.

Qualifications:

• Bachelor's degree in a healthcare-related field, such as nursing or social work.
• Knowledge of rehab services and programs.
• Excellent communication and interpersonal skills.
• Strong organizational and time management abilities.
• Ability to work well in a multidisciplinary team.
• Proficient computer skills.
