This is a placement contract between you and the employer. If the employer hires a candidate you submit, you'll be paid the contract amount.

Baptist Health Nursing and Rehabilitation Center has a full-time opening in the Social Work Department.

Education - 1. Bachelor’s or Master’s degree in social work or a human services field.
Experience - 1. Previous experience in transition/sub-acute care preferred. 2. Two or more years of experience working in a residential facility. 3. Experience in individual/family counseling, group facilitation, admission and discharge planning, and care coordination.
Skills - 1. Individual, family, and group counseling 2. Ability to provide case coordination, admission and discharge planning with an awareness of available community services. 3. Ability to work as part of an interdisciplinary care team 4. Ability to set priorities and work independently 5. Ability to accept and utilize constructive feedback to enhance professional skills 6. Knowledge of the MDS, depression, and anxiety tools desirable

Provide Admission and Discharge services to the residents and families on our rehabilitation unit
Complete admissions paperwork, MDS assessments, and plan safe discharges for residents
Advocate for the resident
Assist the resident with social and economic barriers to discharge
Provide/arrange counseling
Develop care plans
Act as a liaison between resident and community service providers

Equipment to be Used: Computer, fax machine, copy machine, telephone, printer.
